Long-Term Weight & Metabolic Health Maintenance
Ongoing, lower-intensity specialist support to help you keep the improvements in weight, glucose and metabolic health that you have achieved.

- Annual complication screening for people with current or previous diabetes
What to expect
Weight regain and a return of high blood sugar are common after any form of weight loss, including after stopping weight-loss medicines. Planned maintenance helps detect early changes and act on them promptly. Maintenance is not a guarantee, but it gives you a structured way to protect your progress.
Your medicines
Safe adjustment of insulin and other diabetes or blood-pressure medicines is part of the programme and is done only by your doctor. Please do not stop or change any medicine on your own — sudden changes can cause dangerously high or low blood sugar.
Who this programme is not suitable for
- Pregnancy or breastfeeding
- Type 1 diabetes
- Recent severe low blood sugars (hypoglycaemia) or very high blood sugars
- Unstable heart disease
- Advanced kidney or liver disease
- An active eating disorder
- Significant frailty
If any of these apply to you, you will still receive specialist care — through a different, individually planned pathway rather than this structured programme.
